Cancellation policy
Due to the detailed preparation needed before a new patient initial visit, new patient appointments must be canceled or rescheduled no later than 2 weeks before an initial appointment. If you cancel under 2 weeks from the appointment, you will be charged 100% of the appointment fee.
For a non-program follow-up appointment, you need to cancel within 72 business hours. If you cancel under 72 business hours, you will be charged 100% of the appointment fee.
For program patients, if you cancel with fewer than 72 business hours, you will be charged a $200 fee.
Telemedicine
I do offer telehealth appointments for in-state patients. Out-of-state patients need to be seen in person for the initial visit.
We require a copy of a credit card on file and a copy of your health insurance card
I do not take health insurance but a large portion of your lab work or testing can still be covered by your insurance.
Do you take insurance?
I do not take health insurance. This decision was made to be able to spend quality time with patients and their particular needs without the need to satisfy insurance companies. FSA/HSA may cover services. Please check with your FSA/HSA account.
Will insurance cover my lab work?
Insurance may cover your labs or testing, but this does vary according to the insurance company, copays, etc. The patient is responsible for checking their insurance coverage if this is a concern.
